- What specific challenges did his department identify during its unannounced oversight visit to the Beaconvale Frail Care Centre, particularly regarding living conditions, care routines, hygiene, and resident safety and (b) what immediate actions will the department take to ensure that the recommendations made following the oversight visit are fully implemented without delay?

- The following challenges were identified:
- Repair of windows and doors of all bedrooms
- Inconsistent implementation of cleaning program
- Outstanding updated and approved menu
- Lack of psycho-social activities for residents
- Lack of staff training on policies and rights of older persons
- Lack of policies to address allegations of theft by staff as well as residents
- Outstanding compliant Fire and Safety report
- Outstanding Occupational Health and Safety certificate
- The Department finalised a Service Delivery Improvement Plan with identified timeframes with the Board and Management. Progress with the implementation will be monitored.
